Every so often, someone will develop a new problem that’s totally unrelated to previous health issues.

When this happens, the evaluation process starts all over again so we won’t miss any important signs or symptoms that you might be experiencing. This ensures that we are better able to serve you.

But this time, you won’t need to redo the entire health history questionnaire, unless it’s been more than two years since your last evaluation.

After you purchase a Re-Evaluation, you’ll be sent a link to download your Re-Evaluation Form. Please fill it out and email the form back to us so we’ve received it at least 2 days prior to your scheduled phone call. We will send you a reminder email to get your forms in on time, and another email reminder for the day of your scheduled phone call.

Then your practitioner will call you at the pre-scheduled time. Your consultation time is expected to last approximately 30-to-45 minutes. If you require more time than 45 minutes, you’ll be charged $ 37.00 for every additional fifteen minutes.

Payment

You can choose to pay for your first follow-up evaluation either via PayPal Invoicing or by providing your MasterCard/Visa credit card information over the phone to us.

Presently, we are taking your Mastercard / Visa credit card payments over the phone (12/09). Hopefully, my merchant company will clear up the problem we’ve been having online and be able to offer an easy online option again soon.
